New Patient Information
Welcome to our practice! Your first visit with Dr. Reeves will consist of an
oral examination, diagnostic x-rays, periodontal evaluation, and a comprehensive dental
exam. In most cases, cleaning will also be performed.
Your treatment plan with options will be discussed, and financial arrangements
can be made at that time.
If you have current x-rays at another dental office, please let us know so we can
have your records transferred to our office.
New patients will be asked to fill out a registration form, a health history form, and sign a form
that explains our policies. In order to be
as efficient as possible, new patients with Internet access are encouraged to download, print, and
fill out the following forms before coming in to the office. All forms are available below
