We accept cash, checks,
American Express®,
Visa®, or Mastercard®.
Our office also offers an extended payment plan option with prior credit
approval. We will accept assignment of insurance benefits; however, we do require your copayment
or deductible to be paid at the time of service. The balance of your account is your responsibility,
whether your insurance company pays or not. Your insurance policy is a contract between you and
your insurance company. We are not a party to that contract. We can verify your benefits and provide
you with an estimate of coverage for treatment; however, it is your responsibility to know and
understand your policy and coverage. We are not responsible for any changes in your insurance coverage
once an estimate has been provided to you. All accounts
over 90 days will be subject to a finance charge of 1 percent per month, which is an annual rate
of 12 percent.
Usual and Customary Rates: Our office is committed to providing the best
treatment for our patients. Our fees are considered usual and customary for our area. You
are responsible for payment, regardless of your insurance company's arbitrary determination
of usual and customary rates.
Cancellation Policy: When patients give the office advance notice of their
need to cancel a scheduled appointment, this time can then be allocated to patients with
emergency needs. With this in mind, our office requires a minimum 24 hours' notice if an
appointment must be cancelled. If less than 24 hours' notice is given, a 30 dollar fee
will be assessed.
An adult must accompany all minors during the entire length of a minor's appointment.
